I provided a keyworker role for clients with severe and enduring mental illnesses who are co-worked within a Community Mental Health Team. This role involves relapse prevention, medication monitoring, and assessment for provision of services suitable and providing therapeutic intervention. I assessed client’s functional ability in ADL. I also completed risk assessments and monitored risk and mental state of clients. I feel I am a good team member and highly motivated to perform and succeed to the best of my ability. I am able to readily establish a rapport with clients through specific group work such as anxiety management, self-esteem and confidence building, creative activities and lifestyle groups. I feel I have excellent planning skills through providing an Occupational Therapy service, and also maintaining Occupational Therapy and multidisciplinary records. Within my recent post I would occasionally work flexible hours to ensure therapeutic intervention is provided to ensure the client’s needs are met, which shows I am therefore willing to work flexible hours including weekends.I have a lot of experience working with children as an Occupational Therapist, and I have been babysitting children from I was 14 years of age. I work well with children and I am a very responsible person and the safety of children is always paramount with me.

I feel I have a range of skills to offer and I am always willing to further develop these skills with new challenges and experiences within the future.
